Need to get email and telephone number of police officer to do welfare check on wife in picture

Need to get a contact officer name and his email address to provide picture of my wife to do a welfare check. I will need him to go to my wife that is living on market st.
I am wanting to know if she is looking like her picture and is in good health, because she mention concerns to me about her health.
